City of Tempe Provides Information and Call Center Support for Employees

In an effort to provide disease management services to employees dealing with asthma or diabetes and are covered under the City’s PPO Medical Plan, Specialty Disease Management Services, Inc., has announced that it has contracted with the City of Tempe, Arizona.



The program, called Experience Health365…Tempe, provides registered nurse care managers who help members manage their asthma and diabetes, 24x7 call center support, health risk appraisals, and education and information materials.

As part of the offered services, Experience Health365 nurses work with members to develop a care plan, make sure members receive timely and proper medical care for their condition and assist the members in understanding and adhering to their doctor’s treatment plan.



In addition, the RN Care Managers monitor the members’ condition, remind them about the importance of taking their medication and education them about how to live healthy lifestyles.
The City of Tempe has recognized the impact on employees’ quality of life when suffering from chronic health conditions like asthma and diabetes.
